Home from Hospital
with Carers' Resource

Purpose of Role: Home from Hospital is offered to all residents of Harrogate and Craven and is a service supported by volunteers by offering practical help and support to someone who has just been discharged from hospital. This is a time limited intervention
Qualities required:
o Non-judgemental, sympathetic and a good listener
o Ability to encourage confidence and motivation
o Good communication skills
o Be aware of potential risks
o Own transport essential
Support provided:
o Supported and supervised by a named Carers’ Resource staff member
o Induction to Carers’ Resource
o Guidance and ongoing support
Time Commitment: Time allocated per client to be regularly reviewed.
Tasks you may be responsible for :
o Supporting the person back into their own home
o Light domestic tasks – changing light bulbs, testing smoke alarms, shopping and other household jobs
o Collecting prescriptions and posting letters for those with mobility problems
o Checking the client is safe, warm and well
There is no role designed specifically around going shopping for a client, but if this is the one thing that interests you, please get in touch
Expenses: Agreed expenses will be reimbursed
